What is the All-on-4® Treatment Concept?

All on 4 diagram by oral surgeon in Las Vegas, NV.The All-on-4® Treatment Concept is a comprehensive process that will restore your teeth after the patient has reached a point of having no healthy teeth remaining. This system uses an immediate load fixed prosthetic denture device that is supported with the insertion of four dental implants. We are able to use this system to secure dentures in either the top or bottom arch.

The All-on-4® Treatment Concept works by taking your current or newly made dentures which we then permanently attach directly to your jaw bone, we do this by placing four dental implants straight through the denture and into your jaw. This treatment is unlike other implant replacement options, because the All-on-4® Treatment Concept does not require us to bulk up the bone, or to strengthen it with a bone graft. So, even if you have been wearing dentures for years, we can still permanently install your dentures without timely preparatory work.

The All-on-4® Treatment Concept can work with your current bone mass because the posts are longer, allowing them to be placed into more dense bone, and because the implants are tilted at an angle which adds to its stability. Once placed, there will be a time for healing, not just topical tissue healing, but bone healing. We still want the bone to heal, and grow around the implants through a process known as osseointegration, fusing the implants to your bone. After placement, we will ask that the patient eats only soft foods, and use their teeth with care for about three months. After the implants have had time to bond with the bone, the patient can then resume eating the foods that are part of their regular diet.

What is osseointegration?

Osseointegration is a natural process that we are taking advantage of. Your body is willing to incorporate biocompatible materials, including dental implants. Dental implants, such as the one we will be using in your All-on-4® Treatment Concept treatment are made of the metal titanium. Titanium is the ideal metal for surgical repair, it is lightweight, durable and biocompatible. Titanium is not just used in dental implants but in surgical repairs in many various parts of the body, including shoulders, knees, and hips. The natural process of osseointegration is the fusion of the object, in this case a dental implant, and the patient’s bone. Your living bone will heal, growing around the metal, making the metal part of the bone, and a permanent part of your body.

Traditional Dentures vs. Implant Retained Dentures

The All-on-4® Treatment Concept means your dentures will not shift, they will not become loose, and they will not click in your mouth. You will have permanently placed teeth that you can feel confident in.

Immediate load dentures, which are retained with implants offers patients many benefits including:
•  Increased Chewing Ability: With your dentures securely in place, the patient is able to proceed in normal chewing function. They never have to worry if they can eat that ear of corn again. The dentures are firmly in place and will never come out.

•  Increased Confidence: If you ask any patient with traditional dentures what their biggest concern is, they will list how they look in the top of their answers. Many patients fear their dentures coming out, and how their facial appearance is altered while wearing loose dentures.

•  Maintain Bone Mass: What a patient loses a tooth, there is an immediate reaction in bone resorption, or shrinkage. This is due to the bone next receiving the stimulation it needs to be healthy. Placing four dental implants in either the top or bottom arch helps provide the stimulation your bone needs to remain bulked and strong. The patient will not have their facial appearance alter to the traditional collapsed look that is so common in patients with dentures.

•  Ability to Taste: Upper dentures are often designed to cover the roof of the patient's mouth to create suction. Covering the roof of your mouth is quite irritating to most patients. Patient's find that with the roof of their mouth covered, they have a decreased ability to taste.

•  Control: With your denture securely in place, you can have more control of your mouth. Our patients find that they have more control on how their voice sounds, how they chew, and more control of their tongue. This freedom of motion is invaluable.

Dentures are often a necessary step as an alternative to teeth in aging adults. Traditional dentures, though necessary, have never been noted for being easy to work with. Dentures to provide limited function, and are better than no teeth, but the All-on-4® Treatment Concept, can increase a patient’s comfort and function immeasurably.

Traditional dentures require patients to make drastic changes in their day to day activities, you can never just forget that you have them. Most patients who wear dentures are forced to make drastic changes, changes in the foods they eat and changes in how they socially engage. Dentures require specific steps in preparing for bed and in the morning when you wake, not to mention the many hours that you go without teeth altogether while you sleep. •  Difficulty Speaking: One of the first things any dentist will discuss with new denture wearing patients is learning to speak again with your new denture appliance in place. Patients may be asked to recite passages at home, or to practice their singing voice, they need to relearn how to use their voice while keep the denture device in place.

•  Difficulty Chewing: Learning to chew again with a traditional denture device can be a difficult step. Many patients find that they are forced to alter how and what they eat when adjusting to their new adhesive or suction retained denture.

Long Term Success Rate of the All-on-4® Treatment Concept

The research is an ongoing process, but after more than two decades of use, the long term rate of success for All-on-4® Treatment Concept is positive. Published studies show a 99.6% rate of success when using the All-On-4® Treatment Concept in the lower jaw. The upper jaw has a slightly lower, though still very high, rate of success between 97 to 98%. A question that we are often asked is the level of pain following placement. The first few days they are sore, and find that an over the counter medication is necessary. Some patients may even be prescribed a stronger form of pain relief. Adjusting to your implant retained dentures will require some time. You may find it to be initially unusual to chew or speak. With some practice, it will become easier and better. Overall healing will take some time, though overall the All-on-4® Treatment Concept is a less invasive oral surgery procedure compared to other surgical restorations.
